Pursuant to 655 IAC 3.4[3], individuals with disabilities as defined in the Americans with Disabilities Act may apply for modifications in the examination or examination administration. To request accommodations, submit the following documentation to the Iowa Board of Nursing:
1. Form A – Applicant’s healthcare provider completes. Return to the board office by the healthcare provider or by applicant.
2. Form B – Applicant’s nursing school completes if testing accommodations were provided during nursing school. If no testing accommodations were provided during nursing school, the applicant must indicate by signing the bottom of Form B. Returned to the board office by applicant or the nursing school.
3. Personal Statement – Applicant provides a written personal statement explaining the nature of the disability and accommodation(s) requested. This packet includes all three required documents.

Upon receipt of the required materials we will correspond further. Please allow time for the evaluation of your request by the Iowa Board of Nursing and the National Council of State Boards of Nursing.
