Other State Boards

If you would like to obtain a license in another state, contact that state board of nursing for an application and instructions. Contact information for other boards can be found at http://www.ncsbn.org/regulation/boardsofnursing_boards_of_nursing_board.asp.

Complete the Nursys license verification request form and submit to the National Council of State Boards of Nursing with the appropriate fee of $30.00. The verification form is also available through the state boards of nursing or National Council.
